Here’s
How
Persons
aged 18 or above
| 1. |
Obtain
an application form (Form No. LCS
694) at any Hong Kong Public Library or through
the Internet; |
| 2. |
Complete
Parts A and C of the application form; |
| 3. |
Any
applicant who does not currently hold a card* issued
by the Hong Kong Public Libraries should also complete
Part B of the application form; and |
| 4. |
Provide
the following documents for verification:
| a. |
the
applicant’s smart ID card; and |
| b. |
the
applicant’s proof of address (e.g. electricity
bill, water account etc. This only applies if
the person is not a current Hong Kong Public
Library cardholder*). |

guarantor should complete Part D, the GUARANTEE section
of the form; and |
| 5. |
Provide
the following documents for verification:
| a. |
the
applicant’s smart ID card; |
| b. |
the
original copy or photocopy of the guarantor’s
ID card; and |
| c. |
the
guarantor’s proof of address (e.g. electricity
bill, water account etc.). |
