Availability of Library Forms

Library forms are available at all Hong Kong Public Libraries. Forms for “Reporting Loss of Identity Card Allowed for Library Purposes” (LCS 696b) are also available at the Registration of Persons Offices of the Immigration Department.

Readers may download the forms in PDF format below and then submit them in hard copy. To view / print the forms in 'pdf' format, you may need the Adobe Acrobat Reader software application.

Under the Electronic Transactions Ordinance 2000, you may also submit your forms electronically via the Government’s e-Form or by e-mail.

Before using the e-Form, you need to have:

a. e-Form Programme: This is for filling in an e-Form. To download the programme, click here.
b. Digital Certificate: This will enable the electronic signing of your e-Form. To apply the digital certificate, please visit the website of the Hongkong Post Certification Authority at http://www.hongkongpost.gov.hk
