If
a smart ID card is lost, defaced or damaged, the
cardholder should report to any Registration of Persons (ROP)
Office of the Immigration Department within 14 days and apply
for a new one. If the cardholder is outside Hong Kong, the
report can be made within 14 days from the date of arrival
in Hong Kong. For a list of the ROP offices, please click

If
the card has an e-Cert embedded, the cardholder should
request Hongkong Post Certification Authority to revoke the
e-Cert as soon as possible. The cardholder can submit an online
e-Cert Revocation Request through the Hongkong Post Certification
Authority website at
www.hongkongpost.gov.hk. Alternatively, the cardholder
may complete an e-Cert Revocation Form and submit the form
to the Hongkong Post Certification Authority by post, fax
or through the drop-in box available at the ROP offices.
If
the card is registered for library services, the
cardholder may report the loss by obtaining and submitting
a library form (Form No. LCS
696a) at all public libraries or through the Internet.
Alternatively, the cardholder may obtain a library form (Form
No. LCS 696b) at the ROP offices and submit it through the
drop-in box available there.
One-stop Service
For
cardholders who have an e-Cert embedded in their smart ID
card and/or have registered for the library services, they
can enjoy the convenience of one-stop service to report loss
to the Hongkong Post Certification Authority and/or the Hong
Kong Public Libraries by completing the appropriate form available
in the same office at the same time they visit the ROP offices
of the Immigration Department.
If
the smart ID card has an e-Cert embedded:

an e-Cert Revocation Form. |

seal
then deposit the properly completed and signed form
into the designated drop-in box at the office. |
If
the smart ID card has been registered for library services:
| 1. |
complete
a library form (Form No. LCS 696b). |
| 2. |
indicate
on the form whether the cardholder would like to apply
for using the replacement smart ID card for library
purposes. |

seal
then deposit the properly completed and signed form
into the designated drop-in box at the office. |
