Here’s How

Persons aged 18 or above

1. Obtain an application form (Form No. LCS 694) at any Hong Kong Public Library or through the Internet;

2. Complete Parts A and C of the application form;

3. Any applicant who does not currently hold a card* issued by the Hong Kong Public Libraries should also complete Part B of the application form; and

4.
Provide the following documents for verification:

a. the applicant’s smart ID card; and

b. the applicant’s proof of address (e.g. electricity bill, water account etc. This only applies if the person is not a current Hong Kong Public Library cardholder*).

Persons under the age of 18

1. Obtain an application form (Form No. LCS 694) at any Hong Kong Public Library or through the Internet;

2. Complete Parts A and C of the application form;

3. Any applicant who does not currently hold a card* issued by the Hong Kong Public Libraries should also complete Part B of the application form;

4. Applicant’s guarantor should complete Part D, the GUARANTEE section of the form; and

5.
Provide the following documents for verification:

a. the applicant’s smart ID card;

b. the original copy or photocopy of the guarantor’s ID card; and

c. the guarantor’s proof of address (e.g. electricity bill, water account etc.).

* includes Library Card issued by Ex-Urban Council Public Libraries and Ex-Regional Council Public Libraries
