Apply to Use the Smart ID Card for Library Services
Here’s How
Persons aged 18 or above
1. Obtain an application form (Form No. LCS 694) at any Hong Kong Public Library or through the Internet; 2. Complete Parts A and C of the application form; 3. Any applicant who does not currently hold a card* issued by the Hong Kong Public Libraries should also complete Part B of the application form; and 4. Provide the following documents for verification:
a. the applicant’s smart ID card; and b. the applicant’s proof of address (e.g. electricity bill, water account etc. This only applies if the person is not a current Hong Kong Public Library cardholder*). Persons under the age of 18
1. Obtain an application form (Form No. LCS 694) at any Hong Kong Public Library or through the Internet; 2. Complete Parts A and C of the application form; 3. Any applicant who does not currently hold a card* issued by the Hong Kong Public Libraries should also complete Part B of the application form; 4. Applicant’s guarantor should complete Part D, the GUARANTEE section of the form; and 5. Provide the following documents for verification:
a. the applicant’s smart ID card; b. the original copy or photocopy of the guarantor’s ID card; and c. the guarantor’s proof of address (e.g. electricity bill, water account etc.).
* includes Library Card issued by Ex-Urban Council Public Libraries and Ex-Regional Council Public Libraries
